POSITION

The Adult
Cardiothoracic ICU Unit at Hospital and Medical Center plays a critical
role within the Cardiac and Thoracic Service Line of Excellence supporting
it’s “Heart & Lung Institute”, the only stand-alone
lung transplant center in the Nation. Committed to the highest level
of patient satisfaction, our CICU unit is a dedicated 16-bed unit
that supports facilities adult cardiac, vascular and thoracic post
surgical patients and has a nurse to patient ratio of 1:2 or 1:1.

Facility

The PICU is
part of the Children’s Health Center at Hospital and Medical

as we are seeking a qualified Transplant Coordinator who is a member
of the transplant team responsible for coordination of the transplant
process from candidate referral, evaluation, acceptance, transplant
and follow-up. The Transplant Coordinator serves as a critical resource
to staff, multi-disciplinary team and the community caring for transplant
patients. The Transplant Coordinator also demonstrates knowledge and
skills in the care of patients with end-stage pulmonary disease, from
adolescent to adult, and will play a role in the comprehensive evaluation
of patient(s) for transplant. The Transplant Coordinator is responsible
for managing all aspects of patient care from evaluation to transplantation.
This position also maintains UNOS compliance, QA/QI documentation,
and will assist in the Medicare certification process. In addition,
with the program Medical and Surgical Director and Transplant administrator,
the Transplant Coordinator will assist in developing policies and
procedures, order sets and pathways to improve overall quality of
care and program development. Strong skills in critical thinking,
phone triaging, autonomy and decision-making are highly desirable.

The CRS pediatric program provides all plans related
and managed care functions to over 13,000 active members. The program
enrolls over 300 new members per month and processes over 12,000 claims
during the same period.
CRS provides clinic and tertiary services to medically complex pediatric
patients. CRS provides services to enrolled members through subcontracts
with hospitals for inpatient and out patient services both in and
out of State. Our program contracts with over 800 providers covering
the widest array of clinical subspecialties. CRS also encounters over
55,000 patients per year through clinical services and provides 50+

POSITION SUMMARY:
